Using the JEM Corrective Action Notice & Form
Overview
The Corrective Action Notice is a formal document used by JEM Wellness Brands to address employee performance or conduct concerns. It outlines incidents, expectations for improvement, and available support resources. This process ensures accountability while offering employees a clear path to improvement.
When to Use This Form
Managers and Supervisors should issue a Corrective Action Notice when:
- An employee’s behavior or performance does not meet company standards.
- Prior coaching or informal feedback has not led to improvement.
- Documentation is needed for progressive discipline.
Form Components
Corrective Action Notice Letter
- Incident Summary: Details of the performance or conduct issue(s).
- Required Improvements: Specific changes expected from the employee.
- Support Resources: Training, coaching, or tools available to assist.
- Consequences: Potential disciplinary actions if improvements are not made.
- Acknowledgment: Employee and Manager signatures confirming receipt and understanding.
Corrective Action Notice Form
- Employee Information: Name, ID, Job Title.
- Type of Action: Verbal, Written, Final Warning, or Other.
- Previous Notices: Documentation of prior corrective actions (if applicable).
- Next Steps: Clear resolution plan and expectations moving forward.
Submission & Documentation Process
- Complete the Notice and Form
- Fill out both documents with detailed and factual information.
- Review with the employee in a formal meeting.
- Submit via Zendesk
- Upload the signed documents to Zendesk for HR and Manager approval.
- Upload to Paychex
- Once finalized, upload the signed form to the employee’s profile in the Paychex CSW Website under “Documents.”
Important Notes
- The employee’s signature confirms receipt and understanding, not necessarily agreement.
- Failure to meet expectations may result in further disciplinary action, including termination.
- This form is part of JEM’s commitment to fair and transparent performance management.
