Using the JEM Employee Performance Improvement Plan (PIP) Form
Overview
The JEM Employee Performance Improvement Plan (PIP) is a formal document used to address and support employee performance concerns. It outlines clear expectations, measurable goals, and a structured timeline for improvement. This process is designed to help employees succeed in their roles while maintaining accountability and transparency.
When to Use This Form
Managers and Supervisors should initiate a PIP when an employee’s performance does not meet established expectations. The form should be used to:
- Document specific performance concerns
- Set clear performance and behavior standards
- Outline measurable improvement steps
- Provide support resources (e.g., training, coaching)
- Schedule check-in meetings and a final review
Form Sections
The PIP form includes the following key components:
- Employee Information – Name, job title, location/brand, manager/supervisor, and review period (30/60/90 days or custom).
- Performance Concerns – Specific areas where expectations are not being met.
- Expected Standards – Clear performance and behavior expectations.
- Improvement Steps – Specific, measurable actions the employee must take.
- Support Plan – Training, coaching, tools, and resources provided.
- Evaluation Timeline – Scheduled check-in meetings and final review date.
- Acknowledgment – Signatures from both employee and manager confirming understanding and agreement.
Submission & Documentation Process
- Complete the Form – Fill out all required fields and review with the employee.
- Submit via Zendesk – Upload the completed form to Zendesk for HR and Manager approval.
- Track Progress – Document updates and check-in meetings throughout the review period.
- Final Assessment – At the end of the review period, complete the Performance Improvement Plan Final Assessment Form.
- Upload to Paychex – Upload the final signed PIP to the employee’s profile in Paychex CSW.
- Close the Zendesk Ticket – Update HR and the Manager via the original Zendesk ticket with the outcome and next steps.
Important Notes
- The PIP is a confidential document and will be stored in the employee’s permanent employment file.
- Failure to meet the outlined goals may result in reassignment or disciplinary action, up to and including termination.
