General Liability Incident Form – Crunch Fitness, European Wax Center, Massage Envy
Overview
This form is used to document general liability incidents across all brand locations:
Crunch Fitness (CF), European Wax Center (EWC), and Massage Envy (ME).
It ensures timely reporting, accurate documentation, and appropriate follow-up for incidents involving guests, team members, or third parties.
When to Use This Form
Use the General Liability Incident Form to report incidents such as:
- Guest or employee injuries
- Property damage
- Slip, trip, or fall events
- Any situation that may result in a liability claim
⚠️ Note: This form is not for Workers’ Compensation claims. Those should be submitted through the designated Workers’ Comp process.
Who Should Complete the Form
- On-site managers or team leads should complete the form immediately following the incident.
- If a manager is unavailable, the most senior team member present should complete it.
Submission Instructions
- Complete the form as soon as possible after the incident.
- Provide clear, factual, and detailed information — avoid assumptions or speculation.
- Attach any relevant documentation (e.g., photos, witness statements).
- Submit the form via Zendesk using the appropriate ticket category.

What Happens After Submission
Once submitted:
- The JEM People Operations Team will review the ticket.
- You may be contacted for additional information or clarification.
- Follow-up actions will be coordinated with the appropriate brand and legal teams.
